If you're due for updates on your Flu, COVID, Hep A, Hep B, DTaP, TDaP, MMR, IPV, HPV, or Meningococcal vaccines, stop by and take care of it. Clinical staff will be on-site to answer any questions about the vaccines and eligibility. SAY Sí Report Card Submission Form:

We now have an online form where you can submit your report cards! You can now upload them directly at saysi.org/reportcards. If you have a printed report card, you may also turn that in at the front office to one of our administrative staff.
